We want you to feel supported in your health journey and confident in using your health plan. That's why we provide free Care Management.

Getting started

You’ll be assigned a care manager who makes sure you get the support and resources you need. Your care manager will also work with you to fill out health related assessments (HRA) to make sure you get the right care. Choose one of the following ways to reach out to us and begin your Care Management experience.

Guiding you through the health care system

Your Care Manager works closely with your primary care provider (PCP) to create a an Integrated Care Plan (ICP) that will manage your health and keep it on track. They can help coordinate your care, make sure you have necessary medical supplies and work with you towards achieving your health and wellness goals.

They’ll also be there for you after a recent hospital stay by assisting you in scheduling follow-up appointments, reviewing changes to your medications and more.

Connecting with your Care Manager

Regular communication with your Care Manager, such as reviewing you an Integrated Care Plan, will happen over the phone.

We encourage you to sign up or log in to your Priority Health member account, where you’ll have the ins and outs of your plan at your fingertips. You also have access to the My Care Management tool, where you can send a secure email to your Care Manager and review your personalized care plan.

If you need support with technology use or navigation, let your Care Manager know.

Extra support

You can access your extra, no-cost plan benefits at any time. If you need assistance on how to use them, your Care Manager can help you. These include:

Transportation services

You have up to 30 one-way trips, limited to 100 miles per trip, per year. Your trips include taking you to and from health-related locations, such as in-network providers and pharmacies

Carallel

Carallel's Care Advocates provide telephonic support and research on topics like health insurance, emotional support, stress management, housing and transportation, and guidance on financial matters and legal concerns. Learn more.

Personal Emergency Response System (PERS)

Take advantage of 24/7 access to medical help in the event of an emergency.

PriorityFlex

Use your monthly allowance toward select utilities, pest control services, healthy food and produce, meal delivery, and over-the-counter (OTC) items. Get the details.

One Pass

Reach your fitness goals with live, digital fitness classes and on-demand workouts. You can also receive one at-home workout kit each year. Learn more.

Priority Health has a D-SNP (HMO) plan with a Medicare contract and a contract with the State Medicaid program. Enrollment in Priority Medicare D-SNP (HMO) depends on contract renewal.
